Meh.

One of the errands Dave and I did yesterday afternoon was me stopping by the local Qwest Diagnostics and having them draw samples for the routine blood work Doc wanted.

This morning as Dave was getting up for dialysis at 5:00, I heard the inkjet of the fax machine. Results were in. I was a bit surprised by the A1c. I was expecting worse. Last June, when he broke the Glucovance Rx into it's components, Scott remarked, "That sure is a lot of glyburide. Let's try a smaller dose." So he halved it. Last January's A1c was 6.7. The current was 7.3. I'm waiting to hear back from his nurse to see if he'll kick the dosage back up to where it had been.

GFR calculated out to 83.91. That's great.

Prior was glucovance 5/500 qid.

Current is (metformin 1000/ glyburide 5) bid.

QID? My goodness. Was he eating four times a day?

20 mg of glyburide isn't uncommon, but if he wants to bump back up, I'd recommend no more often than BID dosing. Perhaps 10 mg with lunch and 5 mg with dinner? If he's tolerating the dose well and isn't going hypoglycemic, then 10 mg bid may work well too.

My biggest concern is the use of the drugs since he's on dialysis. But, if he's on a typical 3x/week schedule, then it's probably not too bad. I'd be more concerned about the metformin than the glyburide at that point though.

I think you misread it. Those are my numbers. "...Me stopping by Qwest Diagnostics..."

Dave's renal disease has nothing to do with diabetes. He's not diabetic at all.
